Our privacy policy policy aims to comply with the new GDPR legislation that begins on 25th May 2018. Our aim is to give you peace of mind and confidence knowing that we will manage your data correctly and explain clearly and in detail how and why we use and collect your data, and your rights. The business Revitalize is owned by Andrea Baxter and her company name is Revitalize. Other self – employed therapists also work from the clinic under their own names. They will have their own privacyPolicy policy and agreements with you.
Andrea Baxter, company name Revitalize, offers Sports and Remedial Massage, Pregnancy Massage, Deep Oscillation Therapy, Manual Lymphatic Drainage (MLD) including CDT, Reflexology, Hopi Ear Candling, Indian Head Massage, Swedish Massage, Deep Tissue Massage, Therapeutic Massage, Slavic Massage, Scar Work, Massage and other therapies.

Andrea, and any other therapist treating you, needs your contact details which include name, address, postcode and D.O.B. so we know who you are, and preferably your email address and phone numbers, so we can contact you and so you can get appointment reminders; and we also need your information about your health, lifestyle and age for treatment to go ahead – filled out on a consultation form or health history record.
We need all this information to provide a safe and suitable treatment, and to allow us to make tailor made treatment plans. We need this information to comply with our professional code of conduct and ethics set out by the therapists’ professional bodies and for insurance purposes. Andrea belongs to the SMTO and follows their code of conduct and ethics. We have a lawful basis for processing your personal information, meaning there is a legal reason to ask for your data because of contractual reasons. You do have the right to not share your personal data with us, but if you do not we are unable to offer you a treatment at Revitalize. You must share your personal information (fulfil your side of the contract) in order for us to carry out treatment (fulfil ours ).
Revitalize will not share your personal health history. Usually we ask you to fill out a consultation form before or at your first treatment. Each therapist will usually ask you to do this for them. We then keep records of those treatments. We keep them locked in a filing cabinet at the clinic.
We must retain records for a period of at least 7 years from the date of your last treatment. In a minor’s case this would be 7 years after their 18th birthday.
At Revitalize, we don’t use your email addresses or mobile phone numbers to send out offers and advertising and don’t share them with anyone else – we use them solely for contacting you about appointments and discussion of treatment options – and to answer questions that clients ask.
Andrea buys in the booking system from Appointy so your names, email addresses and phone numbers are on the online booking system. These are therefore not kept together with the paper record of your date of birth or health history information. Note that you have to create a password for you to register on Appointy as a customer in the first place. You have of course the right to be taken off the Appointy booking system (if you email Andrea requesting she deletes you from it). Appointments are then written down by the therapist in the clinic diary and in the therapist’s own personal diary (including only the client’s name and time of appointment).
