What information do we need prior to treatment

Andrea, and any other therapist treating you, needs your contact details which include name, address, postcode and D.O.B. so we know who you are, and preferably your email address and phone numbers, so we can contact you and so you can get appointment reminders; and we also need your information about your health, lifestyle and age for treatment to go ahead – filled out on a consultation form or health history record.

Explanation of why we need this information

We need all this information to provide a safe and suitable treatment, and to allow us to make tailor made treatment plans. We need this information to comply with our professional code of conduct and ethics set out by the therapists’ professional bodies and for insurance purposes. Andrea belongs to the SMTO and follows their code of conduct and ethics. We have a lawful basis for processing your personal information, meaning there is a legal reason to ask for your data because of contractual reasons. You do have the right to not share your personal data with us, but if you do not we are unable to offer you a treatment at Revitalize. You must share your personal information (fulfil your side of the contract) in order for us to carry out treatment (fulfil ours ).

We will not share your personal health history data

Revitalize will not share your personal health history. Usually we ask you to fill out a consultation form before or at your first treatment. Each therapist will usually ask you to do this for them. We then keep records of those treatments. We keep them locked in a filing cabinet at the clinic.

How long will we store your records

We must retain records for a period of at least 7 years from the date of your last treatment. In a minor’s case this would be 7 years after their 18th birthday.

Email addresses and phone numbers

At Revitalize, we don’t use your email addresses or mobile phone numbers to send out offers and advertising and don’t share them with anyone else – we use them solely for contacting you about appointments and discussion of treatment options – and to answer questions that clients ask.

Appointy Booking System

Andrea buys in the booking system from Appointy so your names, email addresses and phone numbers are on the online booking system. These are therefore not kept together with the paper record of your date of birth or health history information. Note that you have to create a password for you to register on Appointy as a customer in the first place. You have of course the right to be taken off the Appointy booking system (if you email Andrea requesting she deletes you from it). Appointments are then written down by the therapist in the clinic diary and in the therapist’s own personal diary (including only the client’s name and time of appointment).

If you feel we are not handling your data correctly, you have the right to complain

If you have any questions relating to this policy or the data we hold for you, please contact us in writing by email to andrea@@revitalizeEdinburgh.co.uk or by post to Revitalize, 66 Ratcliffe Terrace, Edinburgh, EH9 1ST

You have the right to complain if you believe we are not handling your data correctly; please use the contact details above.

If you are unhappy with our response to your complaint you should then contact the ICO (independent commissioner’s office)

Covid Awareness

If you are a new client (or are an existing client seeing a different therapist than previously), then you will be contacted in advance to gather your health history, and to sign a Covid information and disclaimer form. (The latter just 24 hours before your appointment.)
If you are an existing client you may be contacted to see what you need from your massage treatment, to update the consultation form of any changes and to get a Covid disclaimer signed, (again just 24 hours max before your appointment) letting us know that you are symptom free.
If you are not well, please cancel your appointment with us, erring on the side of caution. And follow all normal quarantine and self-isolation rules relevant to Covid if Covid symptomatic.
If you have long COVID we need your GP’s permission to treat you. Some GP’s will not charge you for the letter if you write the letter as if it were from them, and make an appointment with them and ask them to sign it… ie you pay for their time and effort in writing it. You can check with your individual surgeries.
You may not come for a massage treatment within 48 hours of receiving a vaccine or if still suffering from its side-effects. Similarly we don’t want to see you if you have the flu, or a heavy cold as they are systemic infections so that is not good for you, plus they are transferable and we don’t want your germs. If you are not sure about anything please speak to your individual therapist. All our mobile numbers and email addresses can be found on this website. If we are with clients we don’t answer our phones so emails or texts are best.
If lateral flow tests become easily available you could self-test 3 days before your appointment with us and again the morning of your appointment. You can photograph your result on your mobile phone and show us or forward the picture.
Please arrive 1 minute before your appointment and stand at the main door and we will come and get you to let you in. Please do not arrive early as we are currently not using the waiting area and are working tightly to time. Doors are kept locked all the time as we are working in the treatment rooms.
Please come not needing to use the toilet if you can, unless you have travelled far. Of course if you are desperate you can use the facilities, but then you will have to wait while we clean them after you!
Please use the hand sanitiser on arrival and before leaving. We clean the treatment rooms thoroughly in between clients. Ventilation is key, and we open doors and windows where possible between clients for at least 20 mins. We have some windows open all the time, and extractor fans to the outside in every room.
Please come with a clean face-covering. Our preference is that you arrive wearing a FRSM Type IIR surgical mask which means it is water resistant. Thank-you.
